Exam services are by appointment
In order to ensure your visit goes smoothly, we work hard to prepare beforehand. We require all patients to register and complete their history and insurance paperwork in advance of scheduling. We know this is a lot of work, but it helps us schedule the correct amount of time so we can stay on schedule. It also helps us better understand how your services must be billed to insurance and to estimate of your out of pocket costs in advance of your visit so you’re not surprised at check-in. This all helps ensure that that your time in our office is focused on your eye care and not on paperwork or financial surprises.
To request an appointment, follow the link below to review key policies and submit your registration forms. Once we receive your registration forms, we’ll reach out by the next business day with a cost estimate and and available time slots for your appointment, or, to collect additional information if needed.
